To obtain the Spleen of Steel, complete the quest of the DeepFatFriars while on a Oxygenarian Ascension path. At its conclusion, they will teach Spleen of Steel to you (instead of Liver or Stomach, which would be pretty useless).
Spleen of Steel allows your spleen to absorb 20 points of damage, rather than 15.
Spleen of Steel cannot be kept as a permanent skill when Ascending.
